Supported community connections
SUPPORTED COMMUNITY CONNECTIONS (SCC) services will provide individuals with experience accessing their community competently, confidently, and safely; expose individuals to a variety of cultural, social, recreational, volunteer, and future employment opportunities available to them in their community; and educate individuals about how to access their community.
Non-medical transportation
NON-MEDICAL TRANSPORTATION (NMT) services will support individuals with door-to-door pick-ups and drop-offs to and from their home and community-based services.
